First of all, the lodestone networks must be eliminated. It kills the excitement, even for RuneScape gold novice players who have to walk one way to reach a specific location. The second reason is that the clan camp needs to be reduced to half the size it is now. It removed a significant (in size) forest and caused the Falador-Rimmington-Port Sarim area to become unrealistically joined together.
Lastly, I think there are risks that are associated with the "wilderness," which is between cities. You could face natural disasters or outlaws as well as other dangers. Jagex attempted to achieve this by adding level 5 highwaymen into the game but they aren't important to anyone over level 10, and pose no danger for anyone smart enough to run.
These dangers would differ based on your skill level, and they are less common when you are at higher levels. They would come from nowhere and fight just you, like previous random incidents. This would likely help to stop botting. But, I would prefer broadening the map. This is very complicated.
Eastern lands are interesting and I believe they have a lot of potential, but before we go into them, I'd prefer to look at Menaphos and Prifidinas. When they want to retcon, it's extremely helpful to keep them so mysterious. They're great, but unnecessary currently unless Jagex truly wants to build an Asian player base. If they ever were released, I'd love to see tie ins with Player-owned Ports, Majharrat, or another race...but no Pandas like in WOW.
Regarding the maps of the world, I like the work they did to complete the edges. There is still room for improvement but getting rid of those ugly lines at the edges transformed them into cheap OSRS gold more well-rounded areas that make more sense.
